In order for you to try-out and make a team, you will have to provide proof of medical clearance by a qualified health care practitioner.

The form must state that you are cleared to participate in athletics and it must have been performed within 6 months of try-outs at the University of Connecticut.

If you click the link below, we can provide you with the form to have your health care practitioner complete. There is also a waiver that you must complete and return upon your arrival.
