    Try-out medical checklist and forms

    STORRS, Conn. -- The UConn baseball team will hold open walk-on tryouts this fall at J.O. Christian Field, head coach Jim Penders announced Friday (Sept. 6). The tryout will be held on Tuesday, Sept. 17 at 5:00 pm.

    All interested students must complete the tryout medical checklist and physical form (including a sickle cell test) that are located at the top of this page. You must use the form that is provided for your physical. In addition, once this step is completed, the candidate must drop off the forms at the baseball office (Room 67, located in the Hugh S. Greer Field House).

    At that point, the candidate will pick up the necessary NCAA compliance paperwork from the coaches, or on the office door, and complete and return to the office prior to Thursday, Sept. 12.

    Walk-on requirements: In order to participate in walk-on tryouts you must be a currently enrolled student at UConn - Storrs (at least 12 credit hours undergraduate and 9 credit hours graduate), meet the NCAA's certification requirements and have a complete physical including sickle cell screening results prior to trying out. This physical and sickle cell screening can be completed at the infirmary on campus or at your own doctor's office. All appointments are to be made and paid for on your own. All paperwork must be turned into the baseball front office.
